I couldn’t see the photo when I opened the iRecord link.
This certainly looks like some sort of glitch in the system rather than a ‘mistake’.
Although I still think the verifier was being rather obtuse in not simply correcting the record. He could easily have changed it to the binomial. (I’ve had serveral iRecord records corrected like that - although, in general, I don’t submit trinomials.
